BackgroundApproximately 30% to 50% of patients with moderate/severe asthma have inadequately controlled disease despite adherence inhaled corticosteroid (ICS)/long-acting β2-agonist (LABA) therapy. Data on prevalence and burden uncontrolled in specialty settings are lacking.ObjectiveTo evaluate the respiratory specialist clinics United States.MethodsAdults physician-diagnosed attending pulmonary allergy self-reported ICS use previous 4 weeks completed an electronic questionnaire including Asthma Control Test St George’s Respiratory Questionnaire. Additional information was collected using case report form.ResultsOf 774 12 clinics, 53% were not well (mean [SD] Test, 14.3 [3.6] vs 22.4 [1.6] well-controlled patients). Among ICS/LABA users, 56% controlled, which increased increasing dose (low-dose 45.7%; high-dose 59.7%). The group reported more illnesses, comorbidities, poorer health-related quality life Questionnaire, 46.1 [18.9] 19.8 [12.9] group). These also had exacerbations (≥1 exacerbation, 68.9% 43.1%) health care resource utilization asthma-related hospitalization, 10.7% 2.7%); 27.3% receiving systemic corticosteroids. Approximately 40% population eligible for step-up ICS/LABA/long-acting muscarinic antagonist triple therapy, 20% biologic therapy.ConclusionSubstantial unmet needs exist among managed States settings, may be addressed by improved patient physician education, better guideline implementation, adherence.
